Refund Policy

Standard Clinical Services Refund Policy

Performance Sports Medicine provides medical, chiropractic, diagnostic, regenerative, nutritional, and other clinical services based on individualized treatment plans developed by our providers. Standard clinical services are paid as they are rendered and are not sold as prepaid treatment packages.

Because payment is collected for services that have already been provided, fees for completed visits, evaluations, treatments, procedures, or other services are generally non-refundable. Patients are responsible only for services received unless otherwise stated in a separate written program or membership agreement.

MVP Membership Cancellation & Refund Policy

The Performance Sports Medicine MVP Membership is a month-to-month membership designed to provide eligible members with preferred pricing and discounts on participating services.

Members may cancel their MVP Membership at any time. Cancellation will take effect at the end of the current billing month in which notice is received. Membership benefits will remain available through the effective cancellation date.

MVP Membership fees are not prorated for partial months and are generally non-refundable once the billing month has begun. Any services received during the membership period are billed separately according to the applicable member rate.

If a billing error or duplicate charge occurs, Performance Sports Medicine will review the account and make any appropriate correction or refund.

Program Cancellation & Refund Policy

Performance Sports Medicine offers customized programs that may include physician-directed peptide therapy, medications, nutrition, exercise guidance, laboratory testing, body-composition monitoring, and other individualized services.

Customized programs require 60 days’ written notice to cancel. This notice period allows time to account for medications, supplies, and other resources purchased or committed specifically for the patient, and when clinically appropriate, to develop a safe transition or discontinuation plan.

Certain therapies may require a transition period longer than 60 days. When this applies, patients will be informed before beginning that therapy.

Program fees associated with medications, supplies, testing, services, or other expenses already provided, ordered, purchased, or incurred on the patient’s behalf are generally non-refundable.
