Privacy & HIPAA Policy
Performance Sports Medicine is committed to protecting the privacy and confidentiality of our patients’ health information. We maintain medical, billing, diagnostic, laboratory, treatment, and other health information in accordance with applicable federal and Texas privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).
Patient information is used and disclosed only as permitted by law for purposes such as treatment, payment, healthcare operations, or when authorized by the patient. We maintain administrative, physical, and electronic safeguards designed to protect protected health information from unauthorized access, use, or disclosure.
Patients have rights regarding their health information, including the right to request access to their records, request certain corrections, and receive information regarding permitted uses and disclosures of their protected health information.
Records Release to Another Physician or Healthcare Provider
Performance Sports Medicine takes the confidentiality of patient records seriously. As our standard office procedure, patients requesting that medical records be sent to another physician, healthcare provider, or third party will generally be asked to complete and sign a written authorization identifying the records to be released and the person or organization receiving them.
Certain disclosures between healthcare providers may be permitted by law for treatment purposes without a separate authorization. Performance Sports Medicine will release records in accordance with applicable HIPAA requirements and Texas law.
Patient Medical Records Request
Patients may request copies of their own medical records by submitting a written request to Performance Sports Medicine by email, fax, mail, or in person.
The request should include the patient’s name, date of birth, the records being requested, and the preferred method of delivery. If records are to be mailed or electronically transmitted, the patient should provide the appropriate mailing address or email address.
Performance Sports Medicine will process requests within the time required by applicable state and federal law. Reasonable, legally permitted copying, media, or mailing fees may apply when appropriate.
